[Register to View] , a leading nonprofit life plan community (continuing care retirement community) in Asheville, is hiring a full-time and salaried MDS/RN Assessment Coordinator for our skilled nursing and rehab facility. Givens Estates Health Center has a capacity of 60 skilled beds and an average Med A census of 12-14.

Reporting to the Health Service Director, the MDS/RN Assessment Coordinator will lead the Interdisciplinary Care Plan Team through the care planning process, ensuring accuracy and timeliness of all associated processes. This position is responsible for ensuring the quality of care by monitoring, assessing, and maintaining records regarding the MDS and Care planning process in compliance with the requirements of Medicare and Medicaid and ensuring compliance with PDPM, Medicare Advantage plans, and Medicaid Managed care.

What you'll do:
  • Lead the Interdisciplinary Care Plan Team
  • Provide the restorative nurse documentation that coincides with that resident's care plan schedule
  • Conduct and coordinate the development and completion of the resident assessment instrument (MDS) in accordance with current rules, regulations, and guidelines that govern the resident assessment
  • In conjunction with the Rehab Director, ensure proper HIPPS coding and ensure all documentation meets the highest standard for resident care and proper reimbursement under the PDPM system
  • Works closely with Rehab director and business office team members to ensure proper coding is maintained and billing is facilitated
  • Ensure proper supportive documentation is present for all MDS and CAA coding
  • Coordinate the completion of CAA's for each triggered item per current regulations
  • Complete the Care Plan that meets current regulatory standards for each resident
  • Complete all assessment components which may include assessments such as fall assessment, cognition assessment, wandering assessments, self-administration of medication assessment, ROM/contracture/functional mobility assessments, pressure ulcer assessment, and bowel & bladder assessment or train other nurses to assist with these assessments and monitor their assessments for accuracy
  • Interact with residents and families during the care planning process to keep them informed of care plan goals
  • Complete all Discharge and Re-entry assessments as required by regulation
  • Maintain the roster of resident assessments to ensure timely completion of all assessments and care planning
  • Maintain and periodically update written policies and procedures that govern the development, use, and implementation of the resident assessment (MDS) and care plan
  • Maintain an ongoing quality assurance program for the resident assessment/ care planning process and the restorative care program
  • Participate in facility surveys made by authorized government agencies
  • Make Medicare eligibility determinations
  • Train licensed staff in PPS documentation (especially new nurses)
  • Train and periodically review accurate documentation with other staff
  • Complete any reports of the resident assessment/care plan and restorative functions as requested
  • Participate in discharge planning as needed
  • Actively participate and accept personal responsibility to ensure the success of the Corporate Compliance Program and enforce HIPAA regulations
  • Attend Givens Estates Health Center meetings and other meetings as assigned meetings
  • Coordinate and Lead the Multidisciplinary Medication Management meetings (M3), Care Plan meetings, and weekly Medicare meetings

What you'll need:
  • An unencumbered North Carolina RN license
  • Passion for helping older adults
  • An ability to work with minimal direct supervision
  • Knowledge of relevant regulations and standards
  • Strong leadership, interpersonal, and organizational skills
  • Two years of experience working as an RN or LPN in a long-term care setting
  • Dependability, collaborative mindset, and has strong attention to detail with a emphasis on accuracy
  • Knowledge of or ability to learn applicable computer systems and software packages
  • Willing to embrace culture change as we transition to a household environment and focus more deliberately on resident-directed care
  • Ability to lead others and have a positive attitude
  • Willing to work closely with team members and family members in a resident-centered environment
